CAN OPTIMAL RESULT BE OBTAINED BY COIL EMBOLIZATION OF MULTIPLE CORONARY ARTERIOVENOUS FISTULAS?

Transcatheter occlusion of coronary artery fistulae has developed into a safe and effective therapy. This procedure avoids the need for open surgical repair and the attendant complications of cardiopulmonary bypass and median sternotomy. The long-term outcome in patients after transcatheter occlusion remains unknown. In a case of 32-years old woman suffering from exertional angina, coronary angiography revealed multipl congenital coronary artery fistula. Transcatheter coil embolization was performed in the middle part of the largest fistula, 3x20 mm and 5x20 mm complex, platinum coils were placed and occluded the fistula. Residuel flow was revealed at the control coronay angiography one month later. Residuel flow was originating from many small fistulas which were getting larger after coil embolization. Coil embolization of the largest fistula is not appropriate technique for management of the multipl coronary artery fistulas.
