THE LATE REPAIR (2 YEARS) OF AN ILIAC ARTERIOVENOUS FISTULA DEVELOPED AFTER DISCAL HERNIA OPERATION: CASE REPORT

Arteriovenous fistulas of iliac artery are seldom detected and present with a wide variety of symptoms. Symptoms are related to the flow and location of the fistula and to the time between onset of disease and diagnosis. Forty years old female patient presented with edema of both legs, emesis and fatigue. Symptoms of congestive heart failure were detected. Chest x-ray showed cardiomegaly. Arterial pulsations of right lower extremity were weak. Murmur was heard at the right lower quadrant of the abdomen. She was operated for L4-5 disc herniation 2 years ago. A fistula was detected between right iliac artery and vein at the aortography. A pseudoaneurysm of 2.5x3 cm in diameters was also observed intraoperatively. While the iliac vein was primarily repaired the artery was interposed using synthetic graft material. Symptoms of cardiac heart failure were disappeared during 3 years following of the patient. Cardiomegaly significantly regressed after operation. Iliac fistulas either spontaneous or traumatic in origin must be treated immediately because they are frequently accompanied with aneurysm formation.
