Lower extremity ischemia is a rare complication with high mortality and morbidity in patients with peripherial artery disease undergoing open heart surgery. Internal mammarian artery is an important collateral pathway that supplies the ilio-femoral region in patients with lower extremity ischemia. This pathway should be reserved in patients who will undergo to open heart surgery and peripherial arterial surgery separately. We discussed the findings of a case that result in loss of an extremity although internal mammarian artery was not used as a conduit, with diabetes mellitus, peripherial artery disease and coronary artery disease.
