Acute respiratory failure due to a mediastinal cyst compressing the trachea and both main bronchuses is a rare complication. On the chest rontgenogram and computerized axiel
tomography of the chest confirmed the cystic nature of the mass in the mediastinum. The patient who had chronic obstrictive pulmonary disease had been given medical treatment to support respiratory functions for ten days prior to surgery. Surgical intervention was undertaken because of increasing respiratory distress due to tracheobroncheal compression. The lesion was approached through a left posterolateral thoracotomy and with incision of the mediastinal pleura, mediastinal cyst was incised and drained. The treatment and results are discussed and recent literature is rewieved.
