The Role of The Contiuous Antegrade and Retrograde Oxygenated Warm Blood Cardioplegia in Myocardial Protection

In this study, we have compared the method of antegrade/retrograde continous warm blood cardioplegia and normothermic cardioplegia nad normothermic cardiopulmonary bypass (group I) (n=15) with antegrad cold crystalloid cardioplegia (group II) (n:15), in patients with ischemic heart disease with good or moderate left ventricular function. The heartbeat in all patients in group I converted to normal sinus rhythm spontaneously after removal of the aortic crossclamp compared with only 33% of the group II. There was no arrhythima, conduction disturbances and electrocardiographic ischemic changes in each group. Perioperative myocardial infarction was not seen in both groups. CPK and CPK-MD serum enzyme levels were similar statistically (p<0.05), but SGOT levels in group I were significantly less than group II (p<0.05) in postoperative 18. hours. Cardiac index and left ventricular stroke work index were significantly higher than before bypass in group I. Our results suggest that antegrade/retrograde continuous warm blood cardioplegia is a safe and effective method of myocardial protection. We prefer this method in patients who has higher operative risk and in the complex procedures.
