34 patients undergoing mitral valve reconstruction with Kay annuloplasty technique were studied. Fifteen were ischemic origin (Group A) and 19 rheumatic origin (Group B). All of the patients have third and fourth degree mitral regurgitation. In addition to Kay annuloplasty procedures aortocoronary bypass graft operations were performed in all of the patiens in group A and aortic valve replacement in 5 patients, Kay tricuspid annuloplasty in 7 patients and open mitral commissurotomy in 8 patients in group B.
There was no reoperation in group A but mitral replacement was required in 3 patients in group B. Some degenerative changes were found clearly much more common in group B than group A.
We concluded that in annuloplasty cases mitral regurgitation and reoperation requirement are more common in rheumatic valve disease than ischemic mitral regurgitation because of continuing degeneration process of the primary disease.
