BRIDGING TO HEART TRANSPLANTATION BY MECHANICAL ASSIST SYSTEMS: PATIENT AND DEVICE SELECTION

The use of mechanical circulatory support systems becomes more popular either as a bridging to transplantation or recovery method, or as a destination therapy by permanent device implantation for the end stage heart failure patients. Due to the shortage of donor organs, various mechanical systems are used for bridging to transplantation purposes. However, successful results are possible with proper device and patient selection. The cardiac and extracardiac conditions are too important in patient selection, likewise, the device which fulfils the patient’s clinical needs should be chosen. In recent years, micro axial flow pumps such as DeBakey VAD and Jarvik 2000 are attracting more attention besides popular pulsatile systems like Novacor LVAD, HeartMate LVAD, Thoratec VAD and CardioWest TAH. The idea of “pacemaker implantation” for end stage heart failure patients looks possible by the ease of implantation and post surgical maintenance of those miniature devices for “reverse modeling” and “bridging to transplantation” purpose
