Surgical Theraphy by Using Retrograde cerebral Perfusion and Total Circulatory Arrest in a Case Having Infective Endocarditis due to Supravalvulary Aortic Stenosis

This report describes a 4 year old boy who presented with infective endocarditis involving the ascending aorta and the arch vessels with supravalvular aortic stenosis as the underlying pathology. The patient was treated surgically because of the embolic potential of the vegetations inside the ascending aorta. Retrograde cerebral perfusion was utilized in conjunction with hypothermic circulatory arrest to flush particulate materials from the arch vessels during surgery. The vegetations were removed and the supravalvular aortic stenosis was relieved with a bifurcated patch. This case is the only case we could find in the English literatüre describing supraval vular aortic stenosis complicated with infective endocarditis, treated surgically and the use of retrograde cerebral perfusion in a child.
