Isolated four cuspid aortic valve is a rarely seen congenital anomaly. Patients commonly reach older ages asymptomatically and it is diagnosed coincidently in an echocardigraphic or angiographical study or postmortem autopsy examinations. Some cases, the diagnosis is just made in the operation room. Pulmonary valves are more commonly involved than the aortic valve and usually asymptomatic in four cuspid semilunar valve anamoly. Symptomatic quadricuspid aortic valves show aortic insufficiency signs.
