From 1992 to 1993 20 patients with carotid artery stenosis were operated on at the Departmet of Thoracic and Cardiovaskular Surgery Cerrahpaşa Medical Faculty. There were no postoperative mortality and neurologic morbidity. One patient died of thrombosis of the endarterectomized carotid artery 6 months later. All patients received antiaggregan in the postoperative period. In the hands of experienced surgeons carotid endarterectomy with acceptible mortality and morbidity rates prevents stroke . In order to asses intracerebral vessels angiography should be done. When neuromonitoring is not avaible operation should be done under routine shunting. To avoid early occlusion and late restenosis closure of the arteriotomy with vein patch is recommended
